The loss of a Philippine passport should be immediately reported to the proper authorities as soon as possible. The grant of another passport to a person who has reported the latest issuance lost will only be given due course after a thorough evaluation of the applicant's qualifications and a careful determination of the facts surrounding the loss of the Government-issued passport.

The passport is not considered lost if the booklet is in the physical possession of another individual/office/entity which is known to the bearer (example: passport used as collateral for a loan, passport confiscated by an office in relation to procedural requirements for a particular transaction/undertaking, passport in the possession of the employer/company, etc.). In cases like these, the applicant will be required to execute an affidavit explaining the circumstances how and why the passport booklet was confiscated/kept by the individual/office concerned. The Philippine Consulate will confirm the details of the last passport issued to the applicant and require sufficient proof of Philippine identity before the processing of a replacement passport can be authorized.

 

Lost Expired Passport

  1. The applicant is required to submit all the basic documents for a first-time applicant.
  2. The circumstances surrounding the loss of the expired passport shall be declared in a written affidavit to be subscribed and sworn to at the Consulate.

  

In case of a Lost VALID ePassport

  • Police Report with English translation(It must be filed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application. Original and photocopy required.);
  • Notarized Affidavit of Loss in English (It must be signed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application. Original and photocopy required.); and
  • Lost Passport Application Fee: USD 150.

Note: If a photocopy of the applicant's latest issued passport is not available, on top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.

 

In case of Lost EXPIRED ePassport

  • Police Report with English translation (It must be filed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application. Original and photocopy required.);
  • Notarized Affidavit of Loss in English (It must be signed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application. Original and photocopy required.);
  • Lost Passport Application Fee: USD 150.

In case of renewal of a frequently Lost Passport

  • Police Report with English translation (filed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application);
  • Notarized Affidavit of Loss in English (signed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application); and
  • Lost Passport Application Fee: USD 150.

Note: If a photocopy of the applicant's latest issued passport is not available, on top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.

 

In case passport has been mutilated or damaged

  • Notarized Affidavit of Explanation (signed by parent/s, legal guardian or person authorized by parent/s who will accompany the minor applicant on the day of application); and
  • Passport Application with penalty Fee: USD 150.

Note: On top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.

Note: In all instances, the Philippine Consulate reserves the right to require additional supporting documents to establish the identity and personal particulars of passport applicants in case the passport booklet being renewed is mutilated/bears evidence of tampering, and other extraordinary circumstances evaluated on a case-to-case basis.
